Closing on a mortgage: Mortgage

I want the servicer to remove the prepayment penalty from my payoff statement unless it can produce a signed original loan document that clearly authorizes the fee. I also want a complete copy of the Promissory Note, Closing Disclosure, Security Deed, any riders or addendums, a full payoff breakdown, and a written explanation of when and why the prepayment penalty was added to my account. I believe this is an unauthorized payoff fee and/or loan boarding error after servicing transfers.
