Incorrect information on your report: Credit reporting or other personal consumer reports

As a direct result of a recent data breach, my personally identifiable information was compromised and misused without my knowledge or consent. This led to the appearance of unauthorized accounts, unfamiliar credit inquiries, and inaccurate late payment entries on my credit reportall of which I did not initiate, authorize, or recognize.
