Opening an account: Checking or savings account

I received a postcard in the mail addressed to my maiden name from US BANK regarding a new account. The postcard stated that a new account or product was applied for using my maiden name and current address. When I called the company I had to confirm that the person in question had in fact used my social security number by giving the bank my SS number. The agent stated that they did in fact use my social security number to try to open an account online! I asked for a letter with all of the information they could provide including the location of the request. The agent did confirm that the bank declined the opening of the account.
