Opening an account: Checking or savings account

I received a notification by phone and by mail regarding an application for a checking bank account that I did not submit or authorize. I contacted the bank, and they informed me that I needed to file a report with the credit bureau regarding this matter. I am concerned that someone may have used my personal information without my permission and would appreciate any guidance on the next steps I should take.
